Home Care Services

            Comprehensive family-centered, need-based nursing care is provided to antenatals, postnatals, children, and adults. Emphasis is laid on empowering individuals, families, and community through education. A family folder system is followed to ensure continuity of home care and documentation.

Clinic Services

          1.Combined Clinics

                   Combined Clinics are conducted in the villages, once a month in the smaller villages and alternate weeks in the bigger villages on the scheduled days, in facilities that are offered by the villagers. These clinics are managed by Postgraduate M.Sc. (N) nursing personnel along with nursing tutors and graduate nurses. The patients are assessed and treated as per the standing orders. Those clients who are in need of further care are referred to appropriate outpatient services of CMCH and follow-up services are provided in their homes.

            2.First Aid Clinics

                      First Aid clinics are conducted everyday except Sundays in these villages. High risk antenatal mothers are referred to Combined Care Clinic services offered by the Obstetric & Gynaecology Department of CMC Hospital. The department has initiated the Junior Nurse Practitioner Program in 2004, collaborating with Medicine Unit I, II and Rheumatology Unit.

             3.Geriatric Clinics

                       Geriatric clinic is organized once a month in one of the village to provide care to the elderly attending the Geriatric Club.

            4.Antenatal and Immunization clinics

                      Staff work alongside; the existing Primary Health Centres of areas to provide antenatal and immunization services.

           5.Cancer Cervix Screening Clinic

                      Initiated in 2013 in Puttuthakku Village using VIA (Visual Inspection using Acetic Acid) method and this facility enables screening of women for Cancer Cervix and referring for further management.

Referral Services

          Clients who require further care are referred to LCECU, CMC Hospital, Primary Health Centres, Government Medical College Hospital or Private Hospitals according to the preference of patients. The community health nurses are able to work with other agencies and the community to achieve the objectives of promotion of health, prevention of diseases, early diagnosis, treatment and rehabilitation.

                  Geriatric club started by the Department in one of its villages is an active hub for the elderly of the village who engage in simple physical exercises, traditional recreation place and interact with each other and thereby find meaning in their lives

Student Education

              The nursing students are posted along with the staff and Nursing supervisors for home visits, clinics and special programmes in order to gain experience in Community Health Nursing and also to complete the clinical requirements and to learn about Urban and Rural Health Programmes. Problem Based Learning, Community Based Education, Family Centred Nursing Care studies, Health Education cum action projects and Epidemiological Approach are some of the approaches used for nursing students to learn Community Health Nursing. Fellowship in Family Nurse Practice which is in house educational programme was started by the College of Nursing in the year 2005 and six of the Tutors from Community Health Nursing Department had undergone the training programme. Later on these Tutors have joined the CONCH programme and continued to contribute to the Independent Nurse Practice in the Community.
